Aadhar Card, Passport, Voter ID, Ration Card, Driving License, or any other government-issued ID.
Proof of Applicant's Identity and Address :
Aadhar Card, Passport, Bank Passbook, Telephone Bill, Voter ID, Ration Card, Electricity Bill, Gas Bill, Driving License, Rent Agreement, or any other government-issued ID.
Affidavit :
Death Certificate (if applicable) :
If the death occurred in a hospital or medical facility, a death certificate from that institution.
Cremation/Burial Receipt :
If the deceased was cremated or buried, a receipt from the crematorium or burial ground.
Proof of Birth/Age of Deceased :
Birth certificate, matriculation certificate, or any other document showing the date of birth.
Copy of Ration Card :
Police Report (if applicable) :
If the death was reported to the police, a copy of the relevant report.
SDM Order Copy (for delayed registration) :
If the death is registered after a year, a copy of the previous SDM order.
